Miller blades, straight: easier in newborns, smaller children, vs Mac blades
in bigger kids
Always have sizes one size larger and smaller than the calculated size.
Use an uncuffed tube, unless older than 8 y/o, then used a cuffed tube (5.5 mm
inner diameter)
Preemie/Newborn: Miller 0
1 month-Toddler: Miller 1
18 mos- 8 years: Miller/Mac 2
> 8 years: Mac 3
ETT: mm ID (internal diameter). Increases in increments of 0.5 to max 6.5.
Can estimate size by width of pinky finger nail, or nare
- preemie 2.5
- newborn 3.0
- 1 month 3.5
- 1 year 4.0
- older than 1 year: 4 + 1/4 age (years)
- >=10 y/o: can try the largest 6.5
How far to insert tube: (approx 3 x internal diameter)
kg : depth at gum line (for preemies/nbns up to 3 kg, insert tube to:
weight in kg + 6 cm.
1: 7 cm
2: 8 cm
3: 9 cm
> 1 year old: 12 cm + 1/2 age (years)
